Forms & Process for Attendance to a Workshop/Conference

Not sure what forms you need to fill out to attend a conference?

Step 1 (2 forms needed)

Certificated Staff Request for Attendance to a Conference/Workshop  (click the link)

or

Staff Request: No Conference Fee and/or No Substitute Needed (click the link)

and

PDC Workshop/Event Proposal Form  (click the link)

 

Step 2

After you attend an approved conference/workshop submit the Report of Conference Attendance form to your building PDC Representative within 30 days.

If you attended a conference/workshop that was out of the St. Louis Metropolitan Area and meet the requirements for reimbursement, submit the following form.
